RULES FOR YOUTH PROTECTION CLEARANCES

All Volunteers Must Complete the Following:

 

PSP Certificate (PA State Police)

DPW Certificate (Childline History Clearance)

FBI report only if the person has not lived in PA for at least 2

years OR if they live out of state.

 

The Diocese of Harrisburg will no longer be processing the clearances for St. Peter.  Any person who needs to apply for clearance will have to PROCESS THE CLEARANCE ON THEIR OWN.  St. Peter will REIMBURSE the $20 cost (excluding any money order fee) for the two clearances (PSP and DPW) if the clearances have been filed by October 31, 2008 and when the originals have been submitted to St. Peter Office.  Once the clearances have been filed and returned to the individual and then given to the Office, a check for reimbursement will be cut up to OCTOBER 31, 2008.  After October 31, 2008 it will be up to any individual who would like to help with the Youth of the Parish to obtain their clearances, pay for any costs involved and then submit the originals to the Office for review and processing; St. Peter will NOT reimburse any of the cost of applying for clearance after October 31, 2008.  If you do not have a clearance you will NOT be able to participate in ANY activity involving the youth of the parish.  You will not be able to volunteer with the youth until ALL originals have been turned into the office.

 

HOW TO APPLY FOR CLEARANCE:

PSP MUST BE OBTAINED PRIOR TO DPW – A COPY OF THE PSP MUST ACCOMPANY THE DPW APPLICATION.  THE PSP MAY BE PROCESSED VIA AN ONLINE APPLICATION https://epatch.state.pa.us

Click on Record Check; Click on New Record Check; Click on Accept

 

Write the Control # for reference.   This does not indicate that you have a record – it does indicate that the PA State Police want to verify your name and SS#.   You will receive in the mail a copy of your PSP, indicating No Record or Record.   If you have a record – this will accompany the Certificate.

A copy of the PSP Certificate is to be attached to the Pennsylvania Child Abuse History Clearance application  along with a Money Order  for $10.00 – To: Department of Public Welfare, PO Box 8170, Hbg, PA 17105-8170

                                                                                                        

Complete all of Section 1 -  Make sure the information is legible. Any Previous Names SINCE 1975

 

VOLUNTEERS:  A copy of your processed Criminal Record (is your PSP Certificate) – if your certificate indicates you have a record a copy of the record must be attached to this application.

 

Household Members – LIST EVERYONE YOU HAVE LIVED WITH SINCE 1975 – include a separate sheet if needed.

-          this means if you were born in 1958 or later – you must include your parents and siblings –

-          do not forget to include x-wives or x-husbands

If you have not lived in the State of Pennsylvania for at least two (2) years, you must complete an FBI clearance form.  Contact St. Peter’s office for information.
